We know the financial impact of cancer begins immediately after diagnosis; if you are struggling to cope with the extra costs and are:
- Are aged between 16 and 24,
- Eligible for free NHS treatment within the UK, and
- Have been diagnosed with cancer or a bone marrow failure disorder within the last 12 months.
Then CLIC Sargent can help you with our Registration grant. All you need to do is complete the application form and provide two contact details of your clinicians or cancer nurse specialists. Once we have your application we will get in touch with your named professional to confirm the information in your application. Once we have received this confirmation we will get in touch to understand how you would like your grant paid: by cheque or BACs.
If you are a parent completing this form on behalf of your child aged between 16 and 24, please ensure you have permission to share their information and that you use their details when completing the application.
